Tips on what to check for before printing or submitting an affidavit

Appropriately managing a job is crucial to your performance. Not managing a job, including not submitting a correct affidavit, creates unnecessary and costly delays.

Affidavits should be reviewed and promptly processed. Here's what steps you should take before signing an affidavit:

  • Confirm that you closed the job out in the appropriate manner 

  • Correct, date and time of service or attempts

  • Date and Time of when you received (job was assigned to you) the documents is correct. If not, contact your Relationship Manager.

  • Delivery address is correct

  • Complete description name of person served

  • Check for accuracy, misspellings, grammatical errors, and profanity in your attempts. Affidavits are public record and court officials do review these.

  • Ensure the ONLY attempts on the affidavit are YOUR attempts. If not, contact Relationship Manager.

  • Do you have the summons complete? Some clients require the summons to be completed in addition to the affidavit. Check with your Relationship Manager if you are not sure if a summons is required with your affidavit.

  • Do you have your Field Sheets to review when proofing your affidavits? If not, what do you use to cross check the information on the affidavit?

  • Delays in executing Alternative Service Request affidavits often result in additional “fresh” attempts being required by the process server if requirement are not met.
