Affidavit Vendor Expectations
Timely return of affidavits is required by all Service Vendors.
General Affidavit Turnaround Specifications
Affidavits need to be submitted to our office within 72 hours of service except for State Contract Clients. State Contract Client affidavits are required to be returned within a 24-hour period of service.
Please allow up to 48-72 hours for processing affidavits received via mail in office, or affidavits uploaded via the web portal. Due to office changes, processing times may be extended. If an original affidavit is required, please mail to: 9210 S. Western Ave Suite A3,Oklahoma City, OK 73139
TEXAS OAG AFFIDAVITS
Texas OAG Affidavits must be returned in 72 hours or they will not pay out for service. If we don't receive the affidavit in 72 hours then the commission on the job will be voided out.
All affidavits that do not require electronic signature or originals to be mailed will need to be uploaded via the web portal. Instructions are here.
